Job #3173 – Field Registered Nurse – Bronx and Manhattan, NY

Greenlife Healthcare Staffing is seeking a Field Registered Nurse to serve as Coordinator of Care for a full-time opportunity covering Bronx and Manhattan, NY. In this dynamic role, you’ll manage all aspects of patient care in the home setting, ensuring timely and compassionate services that align with regulatory standards and individualized patient needs.

Position Details:

  • Location: Bronx & Manhattan, NY
  • Employment Type: Full-Time
  • Salary: $108,000 – $110,000 annually

Qualifications:

  • Associate or Bachelor’s Degree in Nursing
  • Active New York State RN license
  • Minimum of 1 year of hospital experience
  • Must be comfortable traveling throughout Bronx and Manhattan
  • Strong clinical judgment, communication, and organizational skills

Key Responsibilities:

  • Conduct in-home assessments and determine service eligibility
  • Complete all documentation using HCHB (Homecare Homebase) software
  • Develop and implement individualized Plans of Care in collaboration with physicians and families
  • Provide skilled nursing care including wound care, injections, medication management, and disease education
  • Supervise home health aides and ensure compliance with regulatory standards
  • Coordinate care across disciplines and communicate with physicians and supervisors regularly
  • Maintain timely and accurate documentation of all services and interventions
  • Review outstanding items daily and prioritize according to patient needs and documentation deadlines
  • Participate in case conferences and quality improvement initiatives
  • Engage in discharge planning and patient education for optimal home outcomes
